Markdown is what you want when a PDF has to become editable text that lives in a repo, a wiki or a docs site. Doclium infers the structure from the page -- what the document treats as a heading, a list, a table -- and writes it out as plain Markdown.
From the page itself: the most common text size is treated as body text, and only meaningfully larger text becomes a heading. The detection is deliberately conservative -- over-eager guessing turns half a document into headings, which is worse than plain paragraphs.
